Banner Hospice of Northern Colorado
Banner Hospice of Northern Colorado provides high-quality, comprehensive, and compassionate end-of-life care for hundreds of patients and families each year. They provide care in patient’s homes, independent or assisted living facilities, and skilled nursing homes. Offering a wide range of end-of-life care options, Banner Hospice provides compassionate, dignified, individualized support regardless of someone’s age or diagnosis. This fund provides for staff education, equipment, and department needs.
Banner MD Anderson Cancer Center at NCMC
MD Anderson Cancer Center, one of the nation’s leading cancer centers, partnered with North Colorado Medical Center in August 2018. Their services include outstanding cancer programs that integrate the latest research, education, and prevention. They believe patient care is most effective when it’s truly collaborative. Their evidence-based, multidisciplinary approach brings together physicians from all cancer specialties to provide highly coordinated and compassionate care. Their cancer experts include medical oncologists, surgeons, radiation oncologists, physician specialists, nurses, and clinical support staff. The Banner MD Anderson Cancer Center at NCMC fund provides for staff education, equipment, and department needs.
Breast Health Center
The fund supports Banner Imaging Greeley Breast Center for equipment, staff education, and department needs.
Bright Futures Colorado
Bright Futures provides grants to Weld County high school graduates, GED recipients, and honorably discharged veterans toward post-secondary education or a trade certification.

Cardiac and Pulmonary Rehab
The Cardiac and Pulmonary Rehab fund provides financial assistance to patients who would otherwise not be able to engage in the rehabilitation program, staff education, and department needs.
CardioVascular Institute
The CardioVascular Institute fund provides for staff education, equipment, and department needs.
Continuing Education for NCMC Employees
Weld Legacy Foundation supports employees at NCMC by granting funds for expenses related to continuing education conferences and seminars. Ongoing staff education enhances job skills and enables staff members to provide higher quality healthcare services.
Critical Care Unit
The Critical Care Unit fund provides for staff education, equipment, and department needs.
Curtis Strong Center for the Visually Impaired
Curtis Strong was long known as a colorful storyteller and reader of poetry. A Colorado native, he was a business owner, avid pilot, and outdoor enthusiast. During his final years of life, he suffered from macular degeneration, an eye disease that caused him to lose most of his vision.
After Curtis Strong passed away in 2000, Weld Legacy Foundation was the residual beneficiary of his estate and received nearly $2.3 million. This funding established the Curtis Strong Center for the Visually Impaired.
Ensight Skills Center now operates the Curtis Strong Center in Greeley. Ensight is accredited by the National Accreditation Council (NAC). The NAC is the standard setting and accrediting organization for agencies serving people with blindness or visual impairment.
The Center’s typical client has a vision loss that cannot be corrected medically or surgically. Most clients struggle, even with corrective glasses, to read the newspaper, pay bills, or see the computer screen. The Center’s purpose is to assist these individuals to maximize the vision they currently possess.
Diabetes Care Coordination
The Diabetes fund provides for patient education, equipment, and department needs.

The Emergency Department fund provides for staff education, educational materials for patients, equipment, and department needs.
Healthcare Providers Support Fund
The Healthcare Providers Support fund provides meals, PPE equipment, and support groups for the healthcare professionals at NCMC.
Heart Safe City
In 2012, Weld Legacy Foundation partnered with the City of Greeley, Greeley Fire Department, Banner NCMC Paramedic Services, and NCMC’s CardioVascular Institute in the Heart Safe City initiative. The Heart Safe City is a community-wide effort to educate citizens on the dangers of sudden cardiac arrest, identify symptoms, learn how to administer cardiopulmonary resuscitation (CPR), and use automated external defibrillators (AEDs).
The Heart Safe City initiative also called for the placement of AEDs in public gathering spots throughout our community. These AEDs are equipped with simple step-by-step instructions. In 2013, Greeley became the second city in Colorado to officially earn the Heart Safe City distinction. Since 2013, over 1,000 community members have been trained in CPR through this program, and many AEDs are now in service throughout Weld County.
Hospitality Closet
The Hospitality Closet provides basic clothing for patients of NCMC who do not have clothing to wear upon discharge because their clothing was damaged or destroyed due to injury or during initial treatment.
Hospitality House
The Hospitality House at NCMC provides lodging for patients who are undergoing outpatient treatments such as chemotherapy or radiation therapy and for family members who need to be near seriously ill or injured loved ones during their hospitalization at NCMC. Hospitality House stays of six to eight weeks or even longer are not uncommon. While the Hospitality House is open 365 days a year, it is non-clinical and not staffed 24/7; guests receiving outpatient treatment at NCMC must have someone stay with them.

Med Evac
Med Evac is an air medical transport service with three bases strategically located in northern and eastern Colorado. Med Evac provides scene and inter-facility transport of sick and injured patients located throughout Colorado, Wyoming, Nebraska, and Kansas. This fund provides for staff education, equipment, and flight suits.
Med-Aid Prescription Assistance Program
The Med-Aid Prescription Assistance Program provides one-time prescription assistance to people in Weld County who have an urgent medication need but no way to cover the cost.
Medical-Telemetry Unit
The Medical-Telemetry Unit fund provides for staff education, equipment, and department needs.
Monfort Children’s Clinic
In 1995, Weld Legacy Foundation responded to the overwhelming need for primary healthcare among children from Weld County’s low-income families and opened a medical clinic. The clinic was designed to provide complete medical care for children who are Medicaid eligible or uninsured.
Named the Monfort Children’s Clinic in honor of the Monfort Family Foundation’s lead gift, the Clinic occupies a 15,000-square-foot building on north 11th Avenue in Greeley built specifically to house a pediatric clinic. In addition to the Monfort Family Foundation’s generous contribution of $1 million, donors in the community contributed nearly $4.5 million to support this important community program. The clinic is owned and supported by Weld Legacy Foundation and operated by Sunrise Community Health.

The Monfort Family Birth Center fund provides for staff education, educational materials for patients, equipment, and department needs.

NCMC Organ Donor Honor Wall
Travis and Jennifer Gillmore, in collaboration with Donor Alliance, Weld Legacy Foundation, and Banner Health North Colorado Medical Center created a dedicated organ donor wall at North Colorado Medical Center with the purpose to educate and promote organ and tissue donation, honor the individuals who courageously donated organs and tissue, and share organ donor and recipient stories. This fund provides ongoing updates and maintenance to the donor wall.
Neonatal Intensive Care Unit
The Neonatal Intensive Care Unit fund provides for staff education, equipment, and department needs.
North Colorado Family Medicine
North Colorado Family Medicine (NCFM) provides family medicine, disease prevention, and chronic illness management for all ages. This fund provides for staff education, equipment, and department needs of NCFM.
Oncology Department
The Oncology Department fund provides for staff education, equipment, and department needs.
Orthopedics
The Orthopedics fund provides for staff education, equipment, and department needs.

The Palliative Care fund provides for staff education, equipment, and department needs.
Patient Support Fund – Breast Cancer
The Breast Cancer Patient Support fund provides breast cancer patients with support for household expenses, transportation, counseling, and massage therapy. This fund also pays for mammograms and diagnostic services and procedures for underinsured women.
Patient Support Fund – Good Neighbor (Indigent Care)
The Good Neighbor Patient Support fund is used most frequently for patients with serious medical conditions. It supports qualifying patients with household needs such as rent, utilities, food, transportation, counseling, massage therapy, and other approved expenses.
Patient Support Fund – NICU and Pediatrics
The NICU & Pediatric Patient Support fund assists with living expenses such as rent or mortgage, transportation to and from the hospital, counseling for anxiety regarding their newborn's health, specialty formula, diapers, car seats, and other approved expenses.
Patient Support Fund – Oncology (Kent McConnell Memorial)
The Oncology Patient Support fund provides support for qualifying patients undergoing cancer treatment with household needs such as rent, utilities, food, transportation, counseling, massage therapy, and other approved expenses.
Pediatric Rehab – Go Baby Go
This fund provides adaptive mobility equipment which can also include assistive technology for children with special healthcare needs. Adaptive equipment can assist with ongoing pediatric rehabilitation and development.

The Pediatrics fund provides for staff education, equipment, and department needs.
Perioperative Services-Surgery
The Perioperative Services-Surgery fund provides for staff education, equipment, and department needs.
Pharmacy
The Pharmacy fund provides for staff education, equipment, and department needs.

Sports Medicine
The Sports Medicine fund provides equipment and supplies to enhance medical care provided to high school athletes.
SOS (Supporting Our Staff)
Known as Supporting Our Staff, the SOS program provides financial support for employees facing a temporary financial hardship resulting from an unexpected personal crisis and is funded by generous employee donations given specifically to this fund.
Weld County Fire and Burn Care Fund
This fund is used for burn garments and other patient needs at Swedish Medical Center out-patient facility at NCMC and grants to support fire prevention services of Weld County Fire Departments and/or Fire Districts.
Wellness Services
The Wellness Services fund provides staff education and education programs for the community.
Will Rothman Family Chaplaincy
The Will Rothman Family Chaplaincy program began in 1995 and was named in honor of Will and Anne Rothman’s generous lead gift.
Staff chaplains at NCMC offer spiritual care to every person who requests it, regardless of faith or cultural background. They counsel those who are facing illness, injury, surgery, trauma, or have learned of a diagnosis that will change their lives. A chaplain is available 24 hours per day to provide spiritual and emotional support to patients and their families and involve local clergy upon request.
Non-profit agencies in Weld County may also request a grant for funding to support needs of their agency.
